General Features of Oral Mucocele
Oral mucocele results from alterations in minor salivary glands due to mucous accumulation.
Can appear in various tissues, including the appendix, gall bladder, and paranasal sinuses.
Characterized by mucin accumulation in connective tissue, leading to swelling.
Two main types: extravasation and retention, without an epithelial lining.
Epidemiology
Common lesion, more frequently seen in children and young adults with peak incidence in the 2nd decade of life.
Extravasation types are more prevalent than retention cysts.
No significant gender predilection.
Typically found on the lower lip, floor of the mouth, and ventral tongue.
Clinical Features
Patients report painless and recurrent swellings, predominantly located on the lip.
Lesions may last from days to years, often draining spontaneously.
Superficial mucoceles can appear as bluish, soft, transparent cystic swellings.
Deeper lesions may be firmer and resemble normal mucosa color.
Pathogenesis of Oral Mucocele
Etiology involves trauma to or blockage of salivary gland ducts.
Extravasation mucoceles arise from leakage of fluid due to trauma.
Retention mucoceles are caused by duct dilation from obstruction.
Mucoceles undergo phases involving inflammation and the formation of a pseudocapsule.
Histopathology
Three distinct morphological patterns of mucoceles noted microscopically.
Mucinous material is often accompanied by vacuolated macrophages, termed muciphages.
Surrounded by granulation or dense fibrous tissue, with herniated ducts sometimes observed.
Presence of inflammatory cells indicates alteration in associated salivary glands.
Oral Mucocele Treatment
Conventional treatment includes surgical removal of the mucosa and affected glandular tissue.
Some mucoceles are transient and may resolve spontaneously.
Larger lesions may require marsupialization to protect surrounding structures.
Efforts are made to remove the cyst intact along with lobules of the salivary gland.
